MWP Community Care is funded by the Australian Government Department of Health and Aged Care through the ‘Commonwealth Home Support programme’.

What is CHSP?

The Commonwealth Home Support Programme (CHSP) is an Australian government initiative aimed at supporting older individuals to live independently and comfortably in their own homes. It provides a range of entry-level support services and assistance to help older Australians maintain their well-being and enhance their quality of life.

The CHSP is designed to cater to people who need some assistance with daily tasks but do not require the level of care provided by the Home Care Packages (HCP) programme. It aims to promote independence, improve social connections, and support overall health and well-being.

MWP Community Care provides the following services through the Commonwealth Home Support Programme:

  1. Social Support: Support and companionship through social activities, outings, and community engagement.
  2. Home Maintenance and Modifications: Minor repairs and modifications to ensure a safe and accessible home environment.
  3. Accompanied Door to Door Transport: Assistance with transportation to medical appointments, shopping, or social events.

To access the Commonwealth Home Support Programme, individuals usually undergo an assessment process conducted by an Aged Care Assessment Team (ACAT) or an Aged Care Assessment Service (ACAS). 

The assessment determines the person’s eligibility for services and helps determine the types and level of support required.

Private Clients

MWP Community Care also provides services for clients that are not on a CHSP Package.

We are able to offer our social support services, exercise and wellbeing calendar and home modification + maintenance services to NDIS, HCP and non-government subsidised clients who would like to stay socially connected and independent in their own home.
